Descripción del Programa:

 

El presente programa tiene por objetivo la dispensación de leches SIN CARGO a afiliados con cobertura del PMI de Swiss Medical Group (SMG).

Para poder acceder a este beneficio los afiliados previamente a concurrir a la farmacia, se dirigirán a la delegación correspondiente de SMG para autorizar la cobertura al 100 % de los productos recetados. Con dicha autorización, el afiliado, deberá concurrir a la farmacia designada al momento de la autorización y entregarla junto a la receta original (dentro de las 48 hs de realizada la autorización y posterior a las 12hs de la misma).

Las funciones de la farmacia serán las siguientes:

  • Por medio de un validador online que le proveerá Farm+ S.A, deberá comprobar el destinatario de la mercadería recepcionada utilizando el ID que se proveerá en la factura de recepción, proveniente de Droguería Del Sud, quien será la encargada de otorgar el pago por prestación en notas de crédito a favor de la farmacia.
  • Deberá recibir y conservar los productos hasta su entrega.
  • Deberá notificar a Farm + en el caso que no se hayan presentado a retirar los productos pasados los 15 días de recibida la mercadería, para así proceder al retiro de las mismas (la retirará Droguería Del Sud).

Como contraprestación por los servicios brindados la farmacia percibirá un importe fijo en nota de crédito provisto por Drog. Del Sud, por cada dispensación realizada.

 

DESCUENTO A EFECTUAR: 100%.

Aclaración: La farmacia NO deberá facturar el producto sino simplemente dispensarlo, recibir y hacer firmar la documentación al afiliado o a quien éste haya designado para el retiro de los productos.

 

IDENTIFICACION DEL AFILIADO:

Para el retiro de los medicamentos el afiliado o tercero designado deberá presentar la siguiente documentación (SIN EXCEPCION):

  1. 1. receta original autorizada por SMG
  2. 2. carnet del afiliado de SMG
  3. 3. D.N.I. (de quién retira).

SE ENCUENTRA TERMINANTEMENTE PROHIBIDA LA DISPENSA DE PRODUCTOS A QUIENES NO PRESENTEN TODOS LOS REQUISITOS DE IDENTIFICACIÓN.

 

LA RECETA DEBERÁ CONTENER LOS SIGUIENTES DATOS:

Nombres y apellido del afiliado, sello y firma con número de matricula del médico prescriptor, fecha de prescripción, productos y cantidad prescripta.

Cabe aclarar que en todos los casos se entregarán sólo los ítems autorizados por SMG.

FIRMA DE LA RECETA Y DATOS

El afiliado o tercero designado para el retiro deberá completar el dorso de cada receta los siguientes datos: Nombre y Apellido, DNI, domicilio, teléfono y Firma.

 

PRODUCTOS A ENTREGAR:

 

MUY IMPORTANTE:

La farmacia únicamente podrá dispensar a los beneficiarios los productos autorizados por la obra social los cuales serán remitidos por la Droguería gerenciadora de este programa: “Droguería Del Sud”.

En el caso de que aun la droguería no haya efectuado la entrega diaria a la farmacia, la misma puede utilizar la mercadería de su stock, y reponerlo con el producto recepcionado por la droguería (esto es posible, ya que no se trata en ningún caso de productos sujetos a trazabilidad, según lo ordena el ANMAT).

 

VENCIMIENTO DEL BENEFICIO:

Una vez recibido el pedido de la Droguería, la farmacia tendrá un plazo de 15 días como máximo  para entregar el pedido. En caso de no realizarse la entrega comunicarse con Farm + S.A. para recibir instrucciones con los pasos a seguir.

CANTIDAD ITEMS (RENGLON) POR RECETA: ítems autorizados por la entidad.

LIQUIDACION:

Al fin de cada mes de consumo, la Farmacia deberá adjuntar:

  1. 1. Recetas originales con su correspondiente autorización y enviarlas a Farm+ S.A.
  2. 2. Si la Farmacia no entrega la receta con su correspondiente autorización, Farm+ S.A. notificará a Droguería Del Sud, la cual procederá a realizarle un debito por la comisión ya abonada.

Presentación: La Farmacia deberá enviar la facturación en forma mensual a la Cámara con su correspondiente DETALLE DE LIQUIDACION.
